Appointments at Clinton Medical Centre can be arranged in person or by telephoning 4822 7788. Please let the receptionist know if you require a longer appointment. Emergencies and urgent cases are always attended without delay.
It would be appreciated if you need to cancel an appointment to contact us as early as possible so that the appointment can be offered to another patient on the cancellation list. Please note that it is our policy that if three appointments are booked and not cancelled or attended (unless there are extenuating circumstances), then we will no longer book appointments for you. If you fail to give us 2 hours’ notice of cancelling an appointment, you will be charged a fee of $15 prior to your next appointment. Payable on the day.
Billing at Clinton Medical Centre is mixed billing. Bulkbilling is only provided to a current Pension or Health care card holders. You will be required to show your current Pension or Healthcare card.
For all other patients the following fees will apply:
15 minute Consultation = $70.00
30 minute Consultation = $140.00
The fee will be charged at the conclusion of your appointment by reception staff and your Medicare rebate will also be processed at that time.
The rebates available to you are:
15 minute Consultation = $38.20
30 Minute Consultation = $73.95
If you are presenting regarding a new or current Workers Compensation claim you need to provide an insurer claim number and insurer name and proof of accepted condition. If you do not have these details, a fee will be charged to you directly on the day of service.
Translation services are available upon request.
New patients attending the Practice MUST have seen the same Doctor for at least 5 consultations before he/she can be provided with any Centrelink certificates or have any form of paperwork completed. All forms and/or paperwork need to be discussed as charges may apply.
Existing patients must see the same doctor for 5 consultations prior to the completion of any paperwork.
Pre-Employment Medicals, Sport Medicals, Commercial Driving Medicals and Housing Forms
These reports are timed and charged according to information required. Please discuss with reception when booking an appointment for these items. We will provide a quote and payment is required on the day. The above Medicals are NOT claimable on Medicare.
Home visits are available to existing patients of the Practice, for those unfit to travel to the Practice and is an integral part of total care. If it is impossible for you to attend the Practice due to severe illness, home visits can be arranged at the doctor’s discretion. Please phone the Practice by 10.00am, when possible to make arrangements for a doctor to visit. We will also visit patients of the Practice in Nursing Homes or Aged Care Facilities. Visits will be conducted within 10kms of the Practice.
Communication/Phone Call Policy
Most questions or concerns are best dealt with by Consultation with the Doctor. Test results will not be given out over the telephone by support staff as information may be misunderstood or misconstrued and is, therefore, best discussed with your doctor. In an emergency the doctor will speak with you immediately. Repeat prescriptions generally require a medical review to ensure the continued accuracy of dose and effectiveness.
Reminder/Recall of Results
Reminder and recall processes are currently available to patients of this Practice. We routinely ensure all necessary reminders and recalls are made in a timely manner. We also participate in the Cervical Screening Register, Mammogram and Immunisation Register.
Require a doctor’s appointment. Patient initiated referrals or backdated referrals without prior consultation with the doctor will not be issued.
Health Information & Management
This practice is committed to providing comprehensive general practice care to all individuals and families in Goulburn and surrounding communities. All patient consultations are kept strictly confidential. Your medical record is a confidential document. It is the policy of the practice to maintain security of personal health information at all times and to ensure that this information is only available to authorized members of staff. You are able to access your patient records on request.
If a third person is to be in attendance (teaching, training or assisting) during your consultation, your permission will be asked prior to this occurring.
